 E-104 form

I am starting a new job in March and my new employer have asked me for a E-104 form. Now, after doing some research and speaking with th HM Revenue and Customs have informed me that this form no longer exists. According to HMRC, this has been replaced by the SEPS041 form. However, I can only get hold of this form if I apply to the foreign authority in Bulgaria and then they will apply to the HMRC.
I've done a Google search for this form, but can't find any information about it. Can anyone that has moved over and works there shed any light on this for me?

Thanks for this. The thing is, I found this and I thought this was the form I had to fill out, but after speaking with HMRC, the guy said to me that I don't and I need to apply for this other form when I'm in Bulgaria through the authorities. In all honesty, I think the guy was completely wrong.
I've contacted my new employer to find out what form they want and said that the E-104 doesn't exist anymore. Hopefully, they'll come back to me!

I've had an email reply from HMRC now and this is what they've said:

Under the provisions of EC Regulations 1408/71, the statement of UK National Insurance contributions was provided on a certificate E104. However, following the introduction of new EU Social Security legislation (EC Regulations 883/2004 &
987/09) from 1 May 2010 we are unable to issue such a statement direct to an individual.

You should contact the foreign institution where you have made your sickness related benefit claim or registration as worker for healthcare and ask that they contact us to request a SED S041 (this has replaced the certificate E104). They should do this by completing SED S040 and sending this request to the following address:

HM Revenue &
Customs
National Insurance Contributions Office
International Caseworker BP1301
Benton Park View
Longbenton
Newcastle upon Tyne
NE98 1ZZ

I trust this has clarified the position.


So now, I need to find out which foreign institution I need to contact. My future employer have said that they don't think the E-104 has been discontinued and are still being as unhelpful. I've now contacted Eures, who are the EU Job mobility portal and once I've got a reply from them, I'm gonna send it to my employer. E-104 form 794030042

Well that sounds straight forward enough - just contact the Bulgarian version of our NI which your employer should pay as you are their employee. Or if you are going to be employed as a health care worker the same thing applies. Maybe your employer doesn't wish to pay NOI (Bulgarian version of NI). Otherwise there should be no problem.
